Abstract
Toxic appendicitis is a severe form of appendicular syndromes where clinical symptomatology is very noisy dominated by deterioration of the general condition with signs of hemodynamic instability. We report the observation of a 4-year-old girl admitted to an array of fulminan purpuras, for whom the etiological report revealed toxic appendicitis.
The treatment consisted of a hospitalization in the intensive care unit for the conditioning and stabilization of his severe sepsis.
Intraoperative exploration revealed a punctate appendage perforated at its tip, with no obvious peritoneal collection. An appendectomy was performed, and three Ascaris's worms contained along the small hail were found and extracted through the appendicectomy port

Key words: toxic appendicitis, child, purpuras fulminans, severe sepsis
